Registration - New Player - Born In Canada

WELCOME!

There is a lot of information here, please read it carefully.   We understand that beginning a new sport can be a bit overwhelming so please feel free to email our Registrar at registrar@coqmha.org with any questions at all.

We are accepting new player paper work but you will not receive a HCR # until July 4th

NEW PLAYER REGISTRATION INFORMATION

Online registration will open at Noon on July 4th

This is only for Players who have never played hockey for an organization before AND were born in Canada.  If you have played hockey for another organization or association, please go to the Register - Transferring Player tab.

If the player was born outside of Canada, please go to the Register - New Other tab.

We have the Mini C program running again this season.  This program is for 4-5 years olds that are new to hockey.  Please click here for more info.  If you wish to register a player for this program you will follow the same steps posted below for all new players which requires a Hockey Canada Account to be set up and the New Player Registration form to be filled out and submitted with a copy of the players Birth Certificate and 2 proofs of residence.

 

Steps for New Player Registration:

Step 1 - Locate and set aside your child’s birth certificate (if your child was not born in Canada please email the Registrar) .

Step 2 - Complete the Player Registration Certificate form

Email the player reg form, copy of the players Birth Certificate and 2 proofs of residence to registrar@coqmha.org.  Once I have everything I will send you the Hockey Canada ID for your player with Registration instructions.

PLEASE NOTE THAT YOUR REGISTRATION IS NOT CONFIRMED UNTIL ALL DOCUMENTS HAVE BEEN RECEIVED

All new player registration will be subject to space availability and space will not be confirmed until documentation is received.

PCAHA PLAYER REGISTRATION CERTIFICATE must be completed by all New Players.

BIRTH CERTIFICATE- New players must provide a photocopy of the player’s birth certificate on a letter size sheet of paper (which will be kept by the Registrar). If you were not born in Canada, please contact the Registrar.

TWO PIECES OF PROOF OF RESIDENCY in Coquitlam are required for all player registrations.   Accepted are property tax notice, Hydro/Gas/Cable bill, drivers license with insurance, or lease/rental agreement. Registration will be rejected unless photocopies of 2 pieces of proof of residency are provided. Please note that all residency documents will be checked again prior to your child's first skate/evaluation even if you have provided it at registration.  If it is not provided, your child will not be allowed on the ice and your registration could be revoked. No exceptions. The residency requirement is mandated by Hockey Canada & BC Hockey & governed by PCAHA & CMHA.   All proofs must be in the player's parent name.

RESPECT IN SPORT – all parents of new CMHA members must sign up for and complete the online Respect in Sports for Parents program.  Please see link under the Risk Mgmt tab above.

 

IMPORTANT REGISTRATION INFORMATION FOR NEW PLAYERS & PLAYERS NEW TO CMHA

Respect in Sport for Parents required for all U6 through U9 and new to CMHA

 

One parent in each family is required to take this one hour online version of Respect in Sport.  The cost is $12.00.  If you have previously taken Respect in Sport - this requirement is waived.

JERSEY DEPOSITS - will be collected by your Team Managers at the beginning of the season.  At the end of the season once the jerseys are returned, the cheques will be shredded.

VOLUNTEER DEPOSIT - All families in CMHA will be required to submit a deposit cheque to their Team Manager (with jersey deposit) to ensure mandatory volunteering hours are fulfilled. Families must use this website to register their chosen tasks. Please see the Volunteering page for more information and email the CMHA Volunteer Director with any questions.
